Jigsaw is introduced as a dying cancer patient who is obsessed with teaching people the value of their own lives. He is considered a "scientist" who is determined to initiate the survival instinct in his "subjects", believing that humanity no longer used its instinct of survival.

Jigsaw is best known for his deadly traps which he usually builds for his subjects, which are often a symbolic representation of what is seen as a flaw in the person's life. The tests are more like "games", first he tells the person the "rules" of the game either by audio or video tape. These rules or tasks the person must perform in order to pass the test and survive; however, these often involve extreme self-mutilation, or serious psychological torture.

The Jigsaw Killer is described as a mysterious murderer who kidnaps people he sees as wasting their lives and putting them through symbolic death traps, being a physical representation of the problems that Jigsaw saw in their lives. As there become more victims, the media name him the "The Jigsaw Killer" - or simply, "Jigsaw" - because he cuts a puzzle-shaped piece of flesh from an unsuccessful victim, a practice reflecting the victim "was missing a vital piece of the human puzzle; the survival instinct".

John Kramer, was originally a toy designer who lived an uneventful life out of laziness and complacency, this results in his wife Jill leaving him, but John remains much the same until discovering he has an inoperable frontal lobe tumor and cancer, a revelation making him realize how valuable and temporary his life is. At the hospital, John witnessed and became distraught at seeing people wasting their lives, knowing his will end soon, and after discovering his successful coworker Paul has cut his wrists he himself makes a suicide attempt and drives a car off of a cliff. This fails, and John instead decides to dedicate the remainder of his life to make people appreciate their own lives by making them face death.

One of his victims, Amanda Young, having survived her trap and sees Jigsaw as a savior, and later becomes his apprentice. She is killed by one of his victims Jeff, who sees his wife shot dead. Jeff then proceeds to slice the dying Jigsaws throat with a power-saw, ending his reign of terror.
